Job Description

This is a Full Time Social Worker position M-F 8am-4:30pm.

Department Description: Case Management is responsible for patient flow through the hospital. Our Medical Social Workers assist patients & families with arranging beds at skilled nursing facilities, as well as other congregate living situations where appropriate. Our team also supports patients and families in navigating their health issues and responses to their illness. The Medical Social Worker also assists the multi-disciplinary team with complex care coordination.
Job Summary:
Investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.
Education, License & Cert:
•LMSW, LCMS, LMHC, LMFC, other accepted licenses as outlined in OMH 599 regulations
•Once Caregiver has obtained Practice Permit, they then year to obtain NY LMSW license.
Essential Functions:
- Provides psychosocial evaluation to meet patient's mental health related needs. Such services include, but are not limited to: Psychosocial Assessment, Risk Assessment, Treatment Planning Diagnosing, Crisis Intervention, collaborates with the team to provide appropriate communication for those requesting medication management, communicates with case manager and other collateral entities, and provides ethical treatment as outlined in OMH 599 regulations

The pay range for this position is $29.04-$45.78 per hour.
