Your Role:

• Perform Comprehensive Nutrition Assessments and interventions
• Deliver Medical Nutrition Therapy (MNT) in alignment with CMS and AHCA standards
• Complete charting for initials, quarterlies, annuals, post-hospitalizations, High Risk and significant changes
• Collaborate with interdisciplinary teams and participate in IDT meetings
• Conduct audits and lead QAPI initiatives
• Offer telehealth consults and document using EMR systems 
• Lead SOC

Qualifications:

• Florida-licensed Registered Dietitian (or RD-Eligible with verification)
• Experience in long-term care preferred not required - willing to train 
• Team-oriented with strong communication and documentation skills
• Passionate about patient-centered, evidence-based care
