Job Description

Hospice Team Coordinator – TC Team Support (Santa Rosa, CA)

LHH is partnering with a leading healthcare organization in Santa Rosa, CA to hire a temporary administrative coordinator to help support TC workflow.

This role is critical to ensuring smooth operations for the TC team, accurate documentation, and effective communication across staff, patients, and families. You’ll work in a fast-paced environment where your organizational skills and proactive approach help the TC team deliver exceptional care.

Key Responsibilities

  • Act as an administrative resource for the TC team.
  • Coordinate schedules, meetings, and follow-ups for team activities.
  • Accurately enter and maintain patient data, including admissions and updates.
  • Ensure all documentation is complete, signed, and uploaded into EMR systems.
  • Generate compliance reports and add coordination notes for team visibility.
  • Serve as a central point of contact for TC team members, staff, patients, and families.
  • Manage incoming calls/messages and route them promptly to the right team member.
  • Facilitate timely processing of physician orders and documentation.
  • Organize and upload visit records, confirm billing and care levels, and manage discharge paperwork.
  • Monitor medical equipment and supply orders; communicate trends/issues to leadership.
  • Assist TC team leadership with meeting prep, materials, and follow-up tasks.
  • Prepare patient-facing materials for facilities as needed.

Qualifications

  • High school diploma or GED required.
  • Minimum 2 years in medical records, medical office support required.
  • Proficiency in Microsoft Office Suite (Outlook, Teams); EMR experience (HomeCare HomeBase preferred).
  • Strong organizational, customer service, and problem-solving skills.
  • Excellent written and verbal communication; ability to maintain confidentiality.
  • Comfortable in a busy, team-oriented environment.

Job Type: Contract (approx. 3+ months)

Start Date: ASAP

Location: Fully on-site, Santa Rosa, CA

Hours: Monday–Friday, 8:00 AM–5:00 PM

Pay Rate: $27–$30/hour (DOE)
